Ordering from LBI Distributors is fast and easy! New accounts are required to complete an Account Set-Up Form. You may print and fill out the form by hand or type directly into it and save a copy to be faxed or e-mailed. Forms should be faxed to (732) 565-9452 or e-mailed to info@lbidistributors.com. Please call us at (732) 565-9456 to confirm our receipt of your form to ensure quick processing. Once you are set up to purchase from LBI, the Product Order Form has all the information you need to place an order. Fill it out completely and fax to (732) 565-9452 or e-mail to info@lbidistributors.com.
